1. VERIFICATION OF DE-ENERGIZED
CONDITION:
Operate the switch controlled equipment &
use voltage tester to make sure all related
power sources are de-energized.
2. BLOCKING VERIFICATION TEST:
If the switch actuator can be moved to the
ON position (after installing the Universal
Wall Switch Lockout) DO NOT use this
lockout.
3. DO NOT lock switch in ON position when
it is unsafe to do so.
4. APPROPRIATE USE of Universal Wall
Switch Lockout is the sole responsibility of
the user in relation to compliance with
OSHA Lockout/Tagout regulations.
5. Keep out of reach of children.
